Measurement key & how to fill this in
1. Employment/Volunteer Placement: This includes vocational assessments, career exploration, vocational training (e.g., job shadowing, mock interviews, resume writing), job development, workplace tours, placement, job coaching within allotted weekly hours, volunteer opportunities/placement (with recognized non-profit), micro-business or self-employment and other related activities as applicable.
2. Post-Secondary Education: This includes supporting individuals in their pursuit of attending College (Community College, 4-year Universities), Adult Education Programs, Continued Education (GED), Trade/Vocational Schools. Individuals will receive training & support to complete the enrollment process, select courses and/or programs, access Disabled Student Services, attend classes, acclimating to the campus, completing course work, assignments and other related activities as applicable.
3. Personal Development/Socialization: This includes (but is not limited to) personal safety training, self-advocacy and assertiveness training, interpersonal relationship development and boundaries, utilizing leisure and recreational activities, weight management and physical exercise programs, conflict resolution, communication skills development, pursuing social opportunities & community connections and any other areas of identified interest.
4. Mobility Training / Transportation Education: In order to promote each consumer's program participation, timely community appointments, etc., Tailor Day Services Trainers will provide mobility skills training and transportation coordination support as often as needed. This includes (but is not limited to) learning bus, train or light rail routes, using local transportation providers (learning how to travel to work sites, college campus sites or community activities of choice).
